Our Role
Following an explosion and tank fire at a large refinery complex, our team was appointed to lead the loss investigation and adjustment. We mobilised immediately, coordinated controlled site access amid parallel official inquiries, and conducted structured interviews with operations, engineering and HSE personnel.
Using plant control data and contemporaneous records, we established a clear sequence of events pointing to a vapour-cloud deflagration originating at one storage tank before fire spread to adjacent tanks. Our reporting distilled a complex technical scene into practical actions that prioritised safety, contained secondary exposures and set the basis for reinstatement.
To drive recovery, we aligned specialists in forensic engineering and accounting, verified stock movements against refinery accounting systems, and ring-fenced allowable heads of cost (including firefighting and debris clearance) within policy terms.
We advised on procurement strategy for like-for-like reinstatement via EPC contracting, and recommended a prudently structured interim payment on account to maintain momentum while causation work continued, balancing expeditious support with robust evidential standards. The approach delivered clarity for stakeholders, reduced downtime, and provided a transparent pathway
from incident to reinstatement.
